APPLICATION PROCEDURE

 Please fill out the application form below. Applications are open from JULY – AUGUST. The completed application form must be brought in to Jamiah Qur’ania Academy in-person or via email along with the following documents (an application cannot be processed until all of the following documents are provided):

 1.    Completed Application form

2.    Official identification of child (passport or birth certificate)

3.    Most recent report card and other relevant school records

4.    Proof of address (recent utility bill: electric, gas, or phone bill)

5.    Most recent Immunization records

After providing all documents, applying students will be scheduled for an interview and will be given a scheduled admissions exam. The administration reserves the right to
accept or reject the applicant.
